Welcome to Smiles By Design, Inc (General and Cosmetic Dentistry) in Little Rock, Arkansas. As a new patient, we will need some information to expedite your first visit to our dental office.
There are two ways to make this easy for you:
- You can print out the New Patient Information Form from our web site, fill it out at your leisure, and bring it with you.
- Or, you can complete our on-line form in full and click “Submit” for us to receive your information in advance of your appointment. We have a secure web site and all of your information will be kept confidential and privileged.
